- Founded in 1998 as a dedicated ambulatory continuity experience
- Staffed exclusively by Med-Peds physicians
- Dedicated staff includes outpatient service representatives, nurses, clinical pharmacists, social workers, medical assistants, referral coordinator, medical interpreters, and cleaning staff
- Offers comprehensive primary care, including preventive care, chronic disease management, and acute care
- Co-locates services for behavioral health, diabetes education, HIV care, long-acting contraception, pediatric cancer survivorship, and resettled refugee intake
- Recognized as a NCQA-Level III Patient-Centered Medical Home
